PLEASE REMEMBER TO BRING THE FOLLOWING:

  • ALL MRI films, reports, CT’s, Bone scans or X-Rays that you might have or have had taken in the past.
  • Allow up to 3 hours once you have arrived at the Doctors office.
  • Consultations can range from $400 – $1050 depending on the amount of X-rays taken at our office.
  • If your insurance is an HMO or a POS please remember to bring your Referral and Doctor Directories in case we need to send you for some more testing or -physical therapy. Knowing who your plan supports will help us to refer within your plan.
  • If you are on a HMO, POS, or PPO that requires a referral and you come to your appointment without your referral we may have to reschedule your appointment for a later date when you have had time to get the proper referral.
